NOTE- Visa applications should be submitted in these 6 authorized CKGS centers only, namely Washington DC, Chicago, New York, San Francisco, Houston and Atlanta.

Applicants are instructed not to go to the Government of India website directly. Application for Indian medical visa is to be filled through the CKGS website.

Once the form in the CKGS website is filled and submitted online, the applicant will be redirected to the Indian Visa Online website.
After the applicant has entered his/her credentials correctly in the preliminary forms at the CKGS website, the list of the documents required for Indian medical visa application will be provided to the applicant.

Important Documents required to fill up the CKGS Application Form
- Click here to visit the CKGS website.
- Current US passport/foreign Passport having a validity period of at least six months from the date of arrival , and at least 2 blank pages. The passport should bear the following information:
- Passport number
- Date of Issue
- Date of Expiry
- Place of Issue
- Details about your previous Indian Visa (if any).
- Details of all the countries you have visited in the last 6 months along with the details of his/her current citizenship.
- Proposed travel itinerary along with the entry and exit points in India and the Particulars of references in U.S and India.
- Other valid passport/identity certificate credentials:
- Passport/certificate number
- Issue date
- Location of issue
- Nationality mentioned there.
After filling the form, click on Proceed and a widget letting the applicant know his/her eligibility of getting an Indian medical visa will appear.
The questions in the form will get auto filled if the applicant is an eligible candidate.
Checklist of documents required to be submitted will be available in the Result Page. Note down the Visa processing time, web reference number that will appear in the same page.
After completing the visa application in the CKGS website, select the “Click Here” button under Fill the Online Visa Form of the Indian Government.
Upon clicking, you will be redirected to Indian Visa Online website.

Step by step process to apply Indian medical visa from USA
- Click on Regular Visa Application when inside the Indian Visa Online
- Fill up the mandatory fields:
- Country’s name
- High commission
- Date of birth
- Nationality
- E-mail id
- Expected date of arrival in India
- Visa type.
Enter the access code and click Continue.
- The online application form consisting three pages will appear. Fill up each page of the form with the correct information.
- Fill all the pages, click Save and Continue at the bottom.
- Enter your family address and details about your profession or occupation.
- Enter all your medical visa application details about the hospital in your country and the hospital in India where you will be having your treatment.
- Enter the duration of treatment, no. of entries, purpose of visit, expected date of journey, expected time of arrival and departure, other information, details about SAARC country visit and reference.
- If you do not have an Indian reference, you can make your medical travel to India through Avocure's assistance .
- You can mention Avocure's office address and contact number of patient relations. You can consult us for filling the form by clicking here.
- Click Save and Continue on the third page, a final preview containing all your details will be displayed.
- Go through them thoroughly.
- Click on Modify/Edit if there is any correction needed. If not, Click on Verified and Continue if all the data entered by you are correct.
- Thoroughly go through the set of instructions that appear on the new window . Press OK for final registration. If you wish to modify details, click Cancel.
- Once you click OK, a window will appear asking you to select the appointment date for submission of application at the Mission Counter. Click on Confirm the Appointment.
- A new window will open after you Confirm the Appointment, bearing two options: either to Print or Save the online application.
- Save the online application. Do not print it immediately as you may lose your application by doing so.
- Click on Print Registered Application and get the hardcopy of your application form.
- Attach the photo box on the first page If you haven’t uploaded your picture in the online application form earlier. The photograph should not be more than 2 inch X 2inch.
- Put your signature on the rectangular box just below the photo box as well as in the box at the bottom of the second page.
- A new web reference number will be given to you by the Indian Online Visa website as soon as you complete your visa application. This reference number will be used to track your visa status in the CKGS website
After getting a print out of the application form, you will be redirected to the CKGS website to complete the rest of the visa application process.
Fill up the supplementary forms and letters as mentioned. You will be required to Self attest and notarize some of the forms and letters as directed.
- Print the Document Checklist.
- Fill your correct particulars in the forms.
- Re-check the forms and letters word to word.
- Print the forms from the ‘My Account’ page.
- Put your signature in the form wherever asked.
- Self-attested or notarized the documents as instructed.
Indian Medical Visa Processing Fee
- The medical visa fee from the U.S to India is US$ 100 for six months and US$ 140 for a year.

Mode of Submission
Shipping and walk in are the two modes of submission of Indian medical visa application.
Shipping
- Shipping is booked and purchased at the CKGS website.
- Send the medical visa application to the CKGS Application Centre of the concerned jurisdiction. Do not sent it to the Mission.
- Print the label having the following:
- To: New ‘Visa’ Application
- CKGS Application Centre name
- Visa Department
- Address
- Location
- Zip Code
Things to remember:
- Only 1 application per package.
- Weight of the package should not be more than 0.5Lbs.
- The shipping label of CKGS should be affixed on the envelope.
Use these three envelopes by UPS and FED EX as shown below:
- Do not opt for pick up services by Fed Ex and UPS as the Cost of pick up by them is not included.
- Do not use the shipping label to submit the medical visa application beyond the applicant’s jurisdiction.
- If the applicant wishes to use his/her own shipping label, they should provide the AWB Number and service provider’s details at the Shipping stage of visa process. It can also be printed later from the ‘My Account’ page.
- FedEx Prepaid Envelopes and USPS Pre-paid labels sent by the applicant are not accepted.
Walk – in
- After Completing the appointment process in the CKGS website:
- Chose an appointment date.
- Select the Time Slot.
- Once the date and time is booked, the others options will be provided
- Get the appointment date and time confirmed from CKGS.
- Submit the application along with all the documents and the visa fees with the concerned CKGS application center at the appointed date and time and
- For applicants of San Francisco Jurisdiction who have opted for walk – in visa application submission, Biometric enrolment for visa application is compulsory.
Applicants having diplomatic passports or UN passport , the ones with no fingers and the citizens below 12 and over 70 years are exempted from it.
Medical Visa Information After Your Arrival in India
Visa Extension:
A period of 12 months is permitted by The government of India but it can further be extended from 12 months depending upon the recovery condition of the patient.
Only if the concerned treatment center recommends this extension or provides a medical certificate, this extension is possible. The recommendation should state his/her ailment, treatment procedure and the number of days he/she would take to recover completely to go back to his/her own country.
To extend his/her stay in the country, The applicant has to go to the FRRO office with the letter and the required documents .
FRRO:
Foreign Regional Registration Office (FRRO) is the primary agency to regulate the registration, movement, stay, departure, and recommendation extension for stay in India. Once arriving in India, Foreigners have to register themselves in the FRRO office.
Below is the required list of the documents for FRRO registration:
- Application form.
- Photocopy of the passport and the initial visa.
- Four photographs of the applicant.
- Details of residence in India.

E-Tourist Visa from the U.S.A to India
If patients require short term medical treatments or check-ups, they can use E-touristvisa for medical travel
Here are the documents that are required for E-tourist visa.
- Scan the first page of the passport and convert it into a PDF version.
- The size of the PDF should be 10KB to 300KB.
- Upload a digital photograph maintaining the following criteria:
- Size: 10KB to 1MB
- The height and width of the photo should be equal.
- Photo should have full face, front view and eyes open.
- Center head should be within the frame.
- Head from top of the hair to the bottom of the chin of the individual should be prominent.
- Background should be plain light colored or white.
- There should be no shadows on the face or on the background.
- There should be no borders in the photo.
